Mr Toufan  BAHRAMI is  senior consultant in cardiac surgery , working at both Royal Brompton and Harefield Hospital, where he treats both NHS and private patients.

He also works at The Harley street clinic in central London.

With more than 4000 cardiac operations, of which 1500 are minimally invasive procedures over past 21 years, he is the one of the only consultant surgeons in the UK expert in all aspects of minimally invasive cardiac surgery including mitral, aortic valve and root and coronary bypass surgery.

He has pioneered and is leading the programme of minimally invasive (keyhole) and endoscopic cardiac surgery at Royal Brompton and Harefield Hospitals. He is also a leading and highly experienced surgeon in complex multiple heart valve and coronary (beating Heart) procedures including heart reoperations over the past 20 years. 

He was previously involved in heart and lung transplantations and mechanical circulatory support which helped him gain expertise in surgery in patients with poor heart function and multiple comorbidities. 

After obtaining the first place in the French National examination for medical specialisation (Concour d’internat), he trained and qualified at Pitié-Salpêtrière University Hospital in Paris, France and gained further training and expertise in London under Professor Sir Magdi Yacoub.

In both France and UK, he trained and worked alongside three world recognized pioneers in cardiac surgery:

  • Professor Alain Carpentier (who pioneered and invented the technique of mitral valve repair as well as inventing the artificial biological valves which are used today worldwide)
  • Professor Sir Magdi Yacoub (who pioneered aortic valve surgery, homograft, stentless valve, and heart and lung transplantation)
  • Professor Alain Cabrol (who did the first heart transplantation in Europe).

Mr Bahrami previously worked as a consultant at The Hammersmith, St Bartholomew's and the London Chest Hospitals.

He has been at Harefield Hospital since 2005.

He has led the minimally invasive and endoscopic cardiac surgery service, including mitral valve repair surgery since 2010. Alongside this, he is an honorary consultant at four other NHS hospitals:

  • Wycombe General Hospital, Buckinghamshire
  • St. Peter's Hospital, Surrey
  • Luton and Dunstable University Hospital, Bedfordshire
  • Epsom Hospital, Surrey.

Mr Toufan Bahrami is nationally and internationally recognised in minimally invasive, endoscopic (keyhole) cardiac surgery including mitral and aortic valve repair and replacement and minimally invasive Coronary artery Bypass surgery (CABG).

He has completed more than 4000 surgical procedures with over 1500 minimally invasive procedures during past 21 years.

He performs approximately 200 procedures per year, including more than 80% minimally invasive procedures and 20% complex multiple valve, beating heart bypass and reoperations (redo surgery) .

He regularly performs procedures on high-risk patients with multiple comorbidities with excellent outcomes. He regularly operates on patients turned down by other institutions.

Mr Bahrami's expertise in cardiac and endoscopic surgery includes:

  • Mitral valve repair - Mr Bahrami trained and worked in France with Professor Alain Carpentier who pioneered the technique. He has an extensive experience in mitral and tricuspid valve repair with more than 1500 Mitral valve procedures performed during his career. He has extensive expertise in re-do mitral valve repair
  • Minimally invasive endoscopic (keyhole) mitral valve repair - pioneering and developing the technique in  London since 2007, with more than 500 minimally invasive repairs he is one of highest volume surgeons in the country.  He performs more than 120 minimally invasive mitral and tricuspid valve repairs per year with a repair rate of 99% and mortality less than 1% (the lowest in cardiac surgery). He trains consultant colleagues in the London Hospitals, the UK and overseas in this field.  He is a recognised trainer for minimally invasive and endoscopic mitral valve surgery by Edwards and BISMICS (British society of minimally invasive cardiac surgery).   He is one of the few selected members of the international endoscopic cardiac surgery club.                                         
  • Beating Heart Mitral valve repair.   Mr Bahrami is the only UK surgeon recognised by Edwards (US) to perform the beating heart Harpoon mitral valve repair, alongside 5 other European institutions. This technique allows repair of the mitral valve without stopping the heart (without a heart/lung bypass machine) through a 4 cm incision on the left side of the chest.
  • Minimally invasive endoscopic coronary artery bypass graft (MIDCAB/CABG). With more than 600 MIDCAB procedures in 20 years, he is the most experienced surgeon in this field in the country. He pioneered the technique of minimally invasive bypass (off-pump) and performed the first worldwide triple bypass operation through keyhole approach in 2005 (published in Annals of Thoracic Surgery)
  • Minimally invasive aortic valve and root replacement (sutureless valve and Resilia valve). He started mini-sternotomy aortic valve replacement in 2003 and the first UK mini thoracotomy AVR in 2005. He has mastered the procedure and performed more than 800 minimally invasive Aortic valve and root (Bentall) operations since then. He performed the first worldwide mini-thoracotomy sutureless aortic valve replacement and the first worldwide Resilia aortic tissue valve implantation. He has trained many consultants in UK and worldwide with this technique and regularly organises courses in Geneva in minimally invasive aortic valve surgery.
  • Endoscopic vein and artery harvesting  (EVH). He has introduced and pioneered the technique of keyhole harvesting of vein and artery for patients undergoing bypass operations in 2006 in the UK.
  • Beating heart Coronary artery bypass operation (Off-Pump CABG). Through a small 10cm incision on the side of the chest while the heart is beating (called off-pump CABG) rather than using the heart and lung bypass machine. He has initiated, developed, and pioneered the technique of beating heart bypass surgery at Harefield since 2001. Since this time, 100% of his isolated coronary (CABG) bypass operations have been performed on the beating heart with no conversion to bypass. He has one of the largest worldwide expertise in this field.
  • Complex Redo operations for multiple valve/bypass/aortic valve surgery. Mr Bahrami has been working in world renowned cardiac surgery centres with the most world-renowned pioneer masters in cardiac surgery since 1994. Having been exposed to most complex procedures, including heart and lung transplantation and artificial heart surgery he has learned how to deal with most challenging operations, including those turned down by other surgeons or institutions. He has an extensive expertise in dealing with patients requiring multiple valve procedures, with poor heart function and multiple comorbidities, and procedures on frail patients. He regularly performs procedures on very high-risk patients for redo surgery.
  • Endoscopic surgical treatment of atrial fibrillationMr Bahrami has developed and pioneered the technique of totally endoscopic atrial ablation. He has conducted the world recognised clinical research trial on surgical versus catheter AF ablation (CASA-AF Trial). He has performed more than 120 of these highly demanding procedures with excellent outcome.

Mr Bahrami gives his patients a very personalised approach to their care. He works as part of a multidisciplinary team (MDT - including a dedicated cardiologist, anaesthetist, intensivist, and surgical practitioners). With the MDT he evaluates every patient to first determine whether they need surgery and then suggests the most appropriate therapy and timing of intervention.

Every patient’s case is meticulously discussed in a highly specialised dedicate MDT (mitral, tricuspid, complex aortic, revascularisation, endocarditis) where a joint decision is made tailored to patients need.

This approached combined with his unique surgical and technical skills associated with the high volume of procedures is the key to achieve the best outcome.

Mr Bahrami is at the forefront of developments in endoscopic minimally invasive, robotic and beating heart surgery. 

Instead of conventional surgery where the breastbone is opened over 20cm, heart valves are replaced or repaired, and bypass procedures are performed by gaining access to the heart through a small incision (about 5 cm) between the ribs. For this he uses a thoracoscope (special camera with 3D view in a special Hybrid theatre) to view the chest cavity and to guide his movements.

He uses the latest state of the art 3D CT scan and TOE with 3D reconstruction to assess perfectly the anatomy and the pathology of the heart to facilitate the operation and to reduce the risks.

These procedures offer the benefits of conventional open-heart surgery but with less traumatic injury (a smaller incision), a shorter stay in hospital, a smaller and more aesthetic scar, and a much quicker recovery time.

Mr Bahrami has been the forerunner in utilising innovative surgical techniques, including:

  • In 2005 he performed the first worldwide three Bypass CABGs through minimally invasive (MIDCAB)
  • In 2007 he performed the first double-lung transplant from a non-heart beating donor.
  • In 2007 he performed the UK's first endoscopic vein harvesting surgery.
  • In 2009 the first worldwide minimally invasive artificial heart implantation 
  • In 2014 he performed the first sutureless aortic valve replacement through a minimally invasive mini-thoracotomy.
  • In 2016 he implanted the first Inspiris Resilia aortic valve using a minimally invasive technique.
  • In 2017, he performed the first worldwide mitral valve repair, valve repair ablation and CABG using a minimally invasive technique.

Mr Toufan Bahrami has published clinical research papers on valve repair (mitral and tricuspid), beating heart bypass surgery, minimally invasive valve and bypass surgery and transplantation as well as endocarditis and reoperations in peer-reviewed journals including the European Journal for Cardio-Thoracic Surgery, the Annals of Thoracic Surgery and the Journal of Thoracic and Cardiovascular Surgery.

Mr Toufan Bahrami has a strong clinical research programme with development of new technology in cardiac surgery including 3D endoscopy in cardiac surgery and robotic assisted heart surgery. He is constantly evaluating his practice by auditing the outcome, adjusting techniques and evaluating new devices by working with Imperial College research department.

He is participating in three multi-centre National trials in mitral valve surgery, TAVI and minimally access aortic valve surgery. He organises regular courses in Geneva for consultants for minimally invasive aortic valve surgery and sutureless aortic valve surgery.
